Integrating public health ethics into shared decision-making for children during the COVID-19 pandemic

This commentary examines how values typically prioritized in public health ethics such as solidarity and justice can be integrated into SDM, where the individual child's best interest and caregiver preferences are often paramount. Additionally, it suggests a framework to integrate public health ethics into the traditional shared decision-making continuum using 4 scenarios that are examined for risks, benefits, settings, and appropriate levels of directiveness.
Is vitamin D deficiency a risk factor for COVID‐19 in children?

Coronavirus disease 2019 (COVID‐19) is a global health problem that can result in serious complications. The aim of this study was to investigate the prevalence and clinical importance of vitamin D deficiency in children with COVID‐19.This study includes 40 patients who were diagnosed to have COVID‐19 and hospitalized with the real‐time reverse transcription polymerase chain reaction method, 45 healthy matched control subjects with vitamin D levels. The age of admission, clinical and laboratory data, and 25‐hydroxycholecalciferol (25‐OHD) levels were recorded. Those with vitamin D levels which are below 20 ng/ml were determined as Group 1 and those with ≥20 ng/ml as Group 2.

Modeling the impact of school reopening on SARS-CoV-2 transmission using contact structure data from Shanghai

Mathematical modeling studies have suggested that pre-emptive school closures alone have little overall impact on SARS-CoV-2 transmission, but reopening schools in the background of community contact reduction presents a unique scenario that has not been fully assessed. This study adapted a previously published model using contact information from Shanghai to model school reopening under various conditions. It investigated different strategies by combining the contact patterns observed between different age groups during both baseline and “lockdown” periods. It also tested the robustness of this strategy to the assumption of lower susceptibility to infection in children under age 15 years.

Childhood immunization and COVID-19: an early narrative review

The COVID-19 pandemic has evolved into arguably the largest global public health crisis in recent history—especially in the absence of a safe and effective vaccine or an effective anti-viral treatment. As reported, the virus seems to less commonly infect children and causing less severe symptoms among infected children. This narrative review provides an inclusive view of scientific hypotheses, logical derivation, and early analyses that substantiate or refute such conjectures. At the completion of a relatively less restrictive search of this evolving topic, 13 articles—all published in 2020, were included in this early narrative review.
Leveraging implementation science in the public health response to COVID-19: child food insecurity and federal nutrition assistance programs

This commentary aims to examine the crucial role of dissemination and implementation (D&I) science—the study of methods to promote adoption and integration of evidence-based research in real-world policy or practice—to improve public health post–COVID-19. D&I science was created for this very situation, in which scientific knowledge is greatly needed but only if it holds practical relevance for the policy, environmental, and organizational systems that advance health. The paper discusses the application of D&I science to rapid evaluations of federal child nutrition assistance programs deployed during the COVID-19 pandemic.

Child abuse and neglect prevention by public health nurses during the COVID‐19 pandemic in Japan

Child abuse and neglect are high‐priority public health issues around the world, but it is known that early care for families with parenting anxiety and stress is essential for preventing abuse (World Health Organization, 2006). In Japan, the country has created a national campaign plan called The Second Term of Healthy Parents and Children 21 (2015‐2024) to address two prioritized agenda: (1) supporting parents with difficulties raising their children; and (2) preventing child abuse from pregnancy. Public health nurses (PHNs) play a crucial role in preventing child abuse and neglect by providing family healthcare in each municipality. In Japan, more than 70% of PHNs work for municipalities or prefectures covering people at various health stages from birth to old age, identifying health issues for infants and their parents before preschool through a variety of health checkups and home visits.
Are child and youth population at lower risk of COVID-19 fatalities? Evidences from South-East Asian and European countries

The coronavirus disease (COVID-19) pandemic has revealed many lacunas of public health preparedness, especially in lower and middle-income countries and fatality differentials between European and South-East Asian countries. The case fatality rate (CFR) in most of the South-East Asian countries is much lower than the European countries. The percentages of child and youth population are more in South-East countries. The study aims to show the impacts of age composition on fatality differentials in European and South-East Asian countries by age-structure, especially the percentage share of child and youth population.

Building trust within and across communities for health emergency preparedness: community engagement for behavioural and social change

Public trust in institutions in all parts of society is critical for health emergency preparedness. Leaders in government, science,public health,the private sector, international organizations, civil society,and the media are charged with identifying potential health risks and developing measures that will minimize their impact. But often, the threats are theoretical, something that may occur in the future, and difficult for many people to grasp as they address their very real day to day needs. It is only through empathy, accurate communications, community partnership, and effective actions that leaders generate the societal investments in resources and energy required to mitigate the effects of potential health hazards.Understanding the importance of public trust in institutions is especially critical during the COVID-19 outbreak,whose containment relies on the cooperative actions of business, NGOs,governments, communities and individuals.
COVID-19 PICU guidelines: for high- and limited-resource settings

Fewer children than adults have been affected by the COVID-19 pandemic, and the clinical manifestations are distinct from those of adults. Some children particularly those with acute or chronic co-morbidities are likely to develop critical illness. Recently, a multisystem inflammatory syndrome (MIS-C) has been described in children with some of these patients requiring care in the pediatric ICU. An international collaboration was formed to review the available evidence and develop evidence-based guidelines for the care of critically ill children with SARS-CoV-2 infection. Where the evidence was lacking, those gaps were replaced with consensus-based guidelines. This process has generated 44 recommendations related to pediatric COVID-19 patients presenting with respiratory distress or failure, sepsis or septic shock, cardiopulmonary arrest, MIS-C, those requiring adjuvant therapies, or ECMO. Evidence to explain the milder disease patterns in children and the potential to use repurposed anti-viral drugs, anti-inflammatory or anti-thrombotic therapies are also described.

Spotlight on child abuse and neglect response in the time of COVID-19

The article calls for adopting a public health approach toward pandemic-related increases in domestic violence ought to be heeded. Adoption of their framework for evaluating and addressing domestic violence and child abuse and neglect can create public health benefits that far outlast the current crisis. School systems and youth-serving organisations can and should play a vital role in addressing the increased abuse and neglect of children during the COVID-19 pandemic.

COVID-19 & Children: Rapid Research Response

UNICEF Innocenti is mobilizing a rapid research response in line with UNICEF’s global response to the COVID-19 crisis. The initiatives we’ve begun will provide the broad range of evidence needed to inform our work to scale up rapid assessment, develop urgent mitigating strategies in programming and advocacy, and preparation of interventions to respond to the medium and longer-term consequences of the COVID-19 crisis. The research projects cover a rapid review of evidence, education analysis, and social and economic policies.
